Diabetes, high blood pressure and high cholesterol rarely hurt at first, which is why many people do not know they have them until a complication appears. At Clínica Hispana Mangum the clinic's medical team helps you measure, understand and control them with regular visits in Spanish or English.

What do we do at your follow-up?

• Lab-based diagnosis: glucose, A1C, lipid panel and kidney function.
• Blood pressure checks at every visit and review of your home readings.
• Medication adjustments based on your results and how you feel.
• A food and activity plan you can follow with your routine and usual meals.
• Foot checks and complication monitoring for diabetes.

What are the usual goals?

For many adults, the American Diabetes Association suggests an A1C below 7% and blood pressure below 130/80. Cholesterol goals depend on your heart risk. Your medical team adjusts each goal to your age and health.

How often should I come?

While treatment is being adjusted, every 1 to 3 months. Once you are at goal, every 3 to 6 months. Bring your glucose meter or blood pressure log so we can review them together.

Signs to get care soon

Blood sugar above 300 several times, blood pressure of 180/120 or higher, chest pain, shortness of breath or weakness on one side of the body. The last three are emergencies: call 911.
