The thyroid is a small gland in the neck that controls how fast the body uses energy. When it runs slow or fast, the symptoms are mistaken for fatigue, stress or weight changes. A simple blood test clears up the doubt.

Which symptoms can come from the thyroid?

Slow thyroid (hypothyroidism)Overactive thyroid (hyperthyroidism)
Fatigue and sleepinessNervousness or anxiety
Weight gainWeight loss without dieting
Feeling cold, dry skin, hair lossFeeling hot, sweating, tremor
ConstipationPalpitations
Heavy periodsLight periods

How is it diagnosed?

With TSH, the main test. If it is abnormal, free T4 and sometimes T3 are added. In some cases a neck ultrasound is ordered to check for nodules.

What is the treatment?

  • Hypothyroidism: a daily thyroid hormone pill. It is taken on an empty stomach with water, 30 to 60 minutes before breakfast and away from calcium or iron.
  • Hyperthyroidism: medications to lower the gland's activity and control palpitations; some cases need a specialist.
  • Follow-up: TSH is repeated about 6 to 8 weeks after each dose change.

Who should get checked?

People with symptoms, pregnant women or those planning pregnancy with a history of thyroid problems, people with relatives who have thyroid disease and patients with type 1 diabetes or other autoimmune conditions.
