Do I get Form I-693 the same day?

Usually not. Some tests, like the tuberculosis test, take a few days. Once all results are in, the civil surgeon signs the form and gives it to you in a sealed envelope.

Do I need the COVID-19 vaccine for the exam?

No. USCIS stopped requiring it in January 2025 and the CDC removed it from its instructions in March 2025. Other vaccines depend on your age and records.

Can I send Form I-693 after the I-485?

Since December 2, 2024 it must be filed together with the I-485; if you send it separately, USCIS may reject the application. Coordinate the exam date with your attorney.

To adjust your status to permanent resident, USCIS requires Form I-693, and only a doctor designated as a civil surgeon can sign it. At Clínica Hispana Mangum this exam is done by a USCIS-designated civil surgeon, with every step explained in Spanish or English.

What does the civil surgeon check?

Your medical history and vaccination records.
A general physical exam.
The tests the CDC requires by age: a tuberculosis blood test from age 2, syphilis for ages 18 to 44 and gonorrhea for ages 18 to 24.
Any vaccines you are missing for your age. The COVID-19 vaccine is no longer required.

How many visits does it take?

Usually two. At the first visit we do the review, draw samples and give vaccines. When results are back, you return for the signed form in a sealed envelope. If the tuberculosis test is positive, a chest X-ray is needed before signing.

What should I bring?

Passport or another photo ID.
Vaccination records, even if they are in another language.
Previous results or X-rays, if you have them.
Your A-Number, if you already have one.

When is the best time?

Since December 2, 2024, Form I-693 is filed together with Form I-485. The best time is when your attorney has the rest of the package almost ready. USCIS only accepts the 01/20/25 edition of the form.

What the civil surgeon does not do

They do not decide your case or give legal advice. If a condition requires a waiver, your attorney handles it.
