What is the I-693 for?
USCIS requires this exam to confirm there are no health conditions affecting your application. It must be performed by a USCIS-authorized physician, known as a civil surgeon.
What does the exam include?

- Required tests, including tuberculosis
- Administration of any missing vaccines
- Form I-693 completed and sealed in the official envelope
Which vaccines do I need?
USCIS requires certain vaccines depending on your age and history (for example, influenza, tetanus, hepatitis, among others). If you're missing any, we can administer them the same day.
What should I bring?

Do not open the sealed envelope. If you open it, the form may be invalidated and you'd have to repeat the process.
